I bought a new phone and reinstalled TW via the QR code. My balance is now $0.00. I put LP into a PancakeSwap farm and cannot see anything now. When I connect to PancakeSwap, it shows nothing. How do I get my coins and tokens back into my wallet?

Screenshot of my wallet address. This is a different address than what I was initially given. What is the purpose of remembering 12 seed words, when after entering them, they do not retrieve my original address, my purchases, and my associated trades?
